The Health and Social Care Partnership (HSCP) is a new organisation that builds on our previous CHCP arrangements. It formally brings together community health services and social work services.

The Health and Social Care Partnership (HSCP) is a new organisation that builds on our previous CHCP arrangements.  It formally brings together community health services and social work services.  This means that if you need support from either or both, you can have an assessment done that will make sure you get the right help, through joined up teams.  

For example, Nurses work closely with Home Helps; Health Visitors work closely with Social Workers; Mental Health staff work closely with the Advice Team – there are all kinds of combinations.

The main point is that the right services are working together to get the right results for people.  The HSCP will work to improve lives by focusing on the outcomes you want to achieve, based on what is most important to you.
